The name Ortiz de Zarate has no logic since the Ortiz de Zarate near Modigliani were Julio Ortíz de Zárate and his brother Manuel Ortíz de Zárate:

Both were Chilean from Italian origin and were not exactly a androgenous model, absolutely opposite to the portrayed in the Modigliani painting.

Maria Blanchard (BIO LINK) was a very close person to Angelina Beloff, wife of Diego Rivera, she even lived in the home of the Rivera family that Modigliani visited regularly, when
Rivera left for Mexico, Angelina stayed in Paris with Blanchard, this is Maria Blanchard in 1909 and near 1932:
